U.S. Government DOT regulations enacted January 2012 require travel sellers to obtain consent from customers prior to receiving a deposit on any travel package that includes airfare. Please fill out the required information below and submit to confirm you have been advised of the following statements: I understand that the Transportation Security Administration (TSA) requires me to carry a government issued identification card in order to board a flight. I have been advised that the name, date of birth and gender that appears on the identification card must exactly match the same such data that is listed on my airline ticket and in my booking records. I acknowledge that my failure to strictly comply with these requirements may result in denied boarding or an undue delay at an airport security checkpoint causing me to miss my flight. I understand that if traveling internationally, I must have a valid passport (some countries may require my passport to be valid for at least 6 months beyond my travel date) and depending upon my destination and nationality, I may need to obtain one or more visas. I understand that the airline tickets or air tours or cruises combined with air that I am purchasing are subject to government and/or government authority tax and fee increases that may be imposed after the date of purchase. I acknowledge that I may be charged additional sums to offset such increased fees. In the event of such an occurrence, Touraid Travel will make an effort to inform me by telephone and/or email. I hereby consent to such post-purchase price increases and authorize Touraid Travel Inc. to charge my credit card for such additional amounts if required. I understand federal law forbids the carriage of certain hazardous materials, such as aerosols, fireworks, and flammable liquids, aboard the aircraft. Changes in temperature or pressure can cause some items to leak, generate toxic fumes or start a fire. Carriage of prohibited items may result in fines or in certain cases imprisonment. For additional information about these restrictions, I can contact my airline directly or visit http://safetravel.dot.gov . I understand that if I am traveling with medical oxygen, liquid oxygen, mobility aids and other assistive devices may require airline pre-approval or be restricted from carriage entirely. Passengers requiring these items should contact their airline operator for information on use of such devices. I understand that some countries require insecticide spraying of aircraft prior to a flight or while passengers are still on the aircraft. In such cases, federal law requires that we refer you to DOT’s disinsection website at http://airconusmer.dot.gov/spray.htm . I understand that if I have purchased an air package with the vendor directly and flights have yet to be assigned, the identity of the carrier, which may include the carrier’s code-share partner, will be assigned and disclosed at a later date.
